License Renewal Checklist

Sea Service: 360 days of service in the last 5 years (documented on CG-719S

USCG Application: Complete and submit CG-719B

Approved Course: If needed, complete a refresher course

Medical Exam: Submit the CG-719K form

• Medical forms should be emailed directly to MEDAIP@uscg.mil

TWIC: Submit proof of TWIC or exemption

Drug Test: Submit current drug screening

Payment: Pay renewal fees via pay.gov and include receipt

Email Submission: Send application to MMCApplications@uscg.mil
